Vanessa Weiland, AGPCNP-BC, HT, MSCP is the founder of Phases Clinic, where she specializes in perimenopause, menopause, and hypermobility care. A Columbia University-trained, board-certified Adult-Gerontology Nurse Practitioner, she brings over a decade of clinical experience to her work as a Menopause Society Certified Practitioner and Certified Clinical Hypnotherapist, combining evidence-based medicine with a whole-person approach to symptom management. Vanessa has a particular focus on the overlap between hypermobile Ehlers-Danlos Syndrome (hEDS) and menopause — an intersection that's often missed in standard care — and works to help patients navigate hormone changes, joint and connective tissue symptoms, and dysautonomia together rather than in isolation.
